Touro's Geriatric Assessment Program Reopens
The Buckman Assessment Program at Touro Infirmary has reopened.
The Buckman
Assessment Program is a consultation service that provides
comprehensive outpatient evaluation of the older adult with
cognitive, behavioral and/or emotional problems.
The entire evaluation includes the following components to
address the patient?s needs:
- neurocognitive evaluation
- psychiatric evaluation
- medical screening and history review
- functional assessment of activities of daily living
- psychosocial assessment
The multidisciplinary diagnostic team meets
with the individual, family and caregivers to review its findings
and recommendations concerning diagnosis, treatment, supportive
services and modifications of daily living.
A summary of the findings and recommendations
is sent to the patient?s primary physician and other appropriate
healthcare providers.
PROGRAM GOALS
Based upon clinical findings, the Buckman Program offers
recommendations that:
Identify an individual?s functional and cognitive abilities
and disabilities;
Promote maximum functional and cognitive status;
Promote quality of life and self-esteem for both patient and
family;
Provide information and referrals to community resources.
TEAM MEMBERS
A multidisciplinary team, experienced in the care of geriatric
patients, conducts the core assessments. It includes specialists
in:
Psychiatry
Psychology
Nursing
Social Work
Occupational therapy
Our medical director is board-certified in both general and
geriatric psychiatry. In addition, specialists in physical
therapy, speech pathology, neurology, and other disciplines
are available for consultation.
FINANCIAL INFORMATION
The Buckman Assessment Program accepts Medicare and supplemental
insurance coverage. Pre-registration and insurance verification
are also offered.
