Touro's Geriatric Assessment Program Reopens

The Buckman Assessment Program at Touro Infirmary has reopened. The Buckman Assessment Program is a consultation service that provides comprehensive outpatient evaluation of the older adult with cognitive, behavioral and/or emotional problems.

The entire evaluation includes the following components to address the patient?s needs:

  • neurocognitive evaluation
  • psychiatric evaluation
  • medical screening and history review
  • functional assessment of activities of daily living
  • psychosocial assessment

The multidisciplinary diagnostic team meets with the individual, family and caregivers to review its findings and recommendations concerning diagnosis, treatment, supportive services and modifications of daily living.

A summary of the findings and recommendations is sent to the patient?s primary physician and other appropriate healthcare providers.

PROGRAM GOALS

Based upon clinical findings, the Buckman Program offers recommendations that:

Identify an individual?s functional and cognitive abilities and disabilities;
